Everyone wants to maintain a youthful look, and are continuously seeking out all the different options available. An increasing number of patients are embracing non-surgical methods for face lifting. Although non-surgical face lifting produce less dramatic results, they offer less downtime, less risk and lower cost.
These treatments are perfect for those who are not quite surgical candidates, having only mild signs of ageing, as well as those who want to delay the sagging before it becomes more serious. Then there are also a number of patients who may be surgical candidates, but are either afraid of surgery or have been put off by some less than favourable outcomes from surgery.
The most well known non-surgical face lift devices utilise ultrasound technology in the form of High Intensity Focused Ultrasound (HIFU). Besides HIFU, there are also other modalities out there that are lesser known, but bring something to the table that HIFU does not.
In this article we will delve into the different types of non surgical face lift treatments, and explain to you the different technologies available. But before that, we need to understand the ageing process of our faces, and the role of non-surgical face lift in delaying or reversing that process.
Collagen and the ageing process
While most people think of ageing as the development of wrinkles, the reality is that the ageing process is much more complicated than that. The ageing process affects the entire facial structure, including skin, subcutaneous fat, muscles and facial bone.
The majority of age related skin changes occur in the middle layer of our skin. Due to ageing and environmental factors, our skin’s natural production of collagen and elastin starts to slow down. In fact, it has been shown that our dermis layer loses 20-80% of its thickness with the ageing process.
Collagen and elastin are the two proteins that keeps our skin plumped, firm and toned. When the amount of collagen and elastin is reduced, our skin takes longer time to assume its original position when pulled or tugged. This gradually leads to skin wrinkles and sag.
Dermal fillers are perfect tools to address loss of volume. Certain dermal fillers such as biostimulators are also indicated for skin tightening to address fine lines, wrinkles, loose and sagging skin.
However, there must be a limit to just how much dermal fillers should be injected, as injudicious and unrestrained use results in Facial Overfilled Syndrome and Pillow Face appearance.
Non surgical face lift delays the ageing process
Non surgical face lift devices work by delivering energy into various layers of our skin to cause what is known as a thermal coagulation zone. These coagulation zones will elicit an inflammatory response and subsequently lead to collagen stimulation and contraction.

What are the non surgical face lift treatments in Malaysia?
The main difference between the different non surgical face lift devices is in how their energy is delivered.
Between 2005-2009, Thermage was the most popular non surgical face lift device. Thermage is an external radio frequency device that produces bulk heating within the skin.
In 2009, Ultherapy was introduced that was based on the principle of High Intensity Focused Ultrasound (HIFU). HIFU is able to create thermal coagulation zones under the skin without affecting the surface.
The Lutronic Genius was introduced in 2019. It utilises the micro needling radiofrequency (RF), that introduced thermal energy within the deep dermis while sparing the outer layers.
HIFU, Ultherapy, Ultraformer III
Both Ultherapy and Ultraformer III are ultrasound based non surgical face lifting devices. HIFU releases highly concentrated ultrasound energy, delivered deep into the skin causing very precise thermal coagulation zones as deep as the Superficial Muscular Aponeurotic System (SMAS) layer.
Both Ultherapy and Ultraformer III are able to deliver energy to three different preset depths, which are 1.5mm, 3.0mm and 4.5mm. Ultraformer III has an extra advantage in that it has a 2.0mm depth transducer for sensitive areas like the eyes. 4.5mm is where the SMAS layer is at. Only HIFU is able to penetrate to this depth, and this is what sets HIFU apart from other non surgical face lift devices.
Ultherapy has the extra advantage in that it provides ultrasound imaging. This allows the doctor to scan and analyse your skin before treatment to avoid complications. With ultrasound imaging, important structures such as nerves and blood vessels can be avoided, providing a safe experience. It also shows where bone is, as ultrasound delivered to bone can be an agonisingly painful experience.
Microneedling RF vs Thermage
Microneedling RF uses radio frequency attached to microneedles to cause similar thermal coagulation zones below the skin, much like HIFU. This is in contrast to traditional external radiofrequency device like the Thermage which works by bulk heating.
Eliminating the bulk heating means there is less unwanted thermal injury to surrounding tissues, thus reducing the risk of complications whilst maximising results. Just recently there have been a serious case of skin burns making headlines due to external radio frequency treatment gone wrong.
Even with traditional microneedling radio frequency devices such as the Infini, physicians would have been delivering energy to the skin without any real time feedback to know just how much energy is reaching the intended target tissues. However, there may be inconsistencies because tissue impedance is variable among individuals. Even in the same person, the tissue impedance might be different from one day to another, and even from one part of the face to another.
